Dr Shan provides multiple methods of accepting patient referrals. To ensure data security, Dr Shan does not accept patient-sensitive clinical information via this website. Please see below the options for referring patients to see Dr Shan. Appointment times and locations will be arranged by Dr Shan's team according to patient preference, place of residence, and clinical urgency.

 

Direct Referral

Dr Shan accepts referrals via email, fax, or phone (for urgent referrals). Dr Shan's team will then contact the patient to schedule an appointment.

Via Practice Management Software

Referrals can also be sent via your practice management software using the integrated SR Specialists & Referrals and HealthShare platforms.

Via Patient

Please provide the patient's name and contact information using the form below. Dr Shan's team will contact the patient to secure an appointment, after which the patient will be instructed to contact you to obtain a referral. This facilitates prompt appointment scheduling while allowing less time-pressured preparation of the referral document.
